With ongoing collaboration, our case managers will formulate an individualized treatment pathway in order to achieve the optimal level of care and financial outcome, while minimizing fragmentation of care.

Care Coordination Consulting will provide both initial and ongoing evaluations to assess the overall healthcare status of each patient, as well as progress with their treatment plan. Evaluations will include current medical status, past medical history, current and future treatment needs, with the goal of coordination of care along the healthcare continuum. Our goal is to provide clients with the maximum value and benefit for their health care dollars, by utilizing preferred providers or through negotiation of rates.
